I wouldn't have thought there was much danger of this happening on a tourist elephant ride. And statistically, I suppose there isn't. But it would still make me reconsider getting on the back of one for fun.

Nasty way to go but we damn well know those animals are tortured into being our play things. I would never support such an activity.
To be honest, I know nothing of the issue of maltreatment of elephants in Thailand or India. (Well, I may have read something about it, but some of the handler/elephant interactions on some documentaries I've seen didn't look a problem to me.)
